This website has one form: the booking request. Here is what happens to what you type in it.

What the form collects

Your name, mobile number, email (if you give it), the reason for your visit and your preferred day. Nothing else is collected by the form, and the site sets no advertising or tracking cookies.

How it is sent

The form is delivered by email through FormSubmit (formsubmit.co), a form-to-email service. Your request is used only to arrange your visit and is not sold or shared for marketing.

Other services on the page

The map loads from Google Maps, and fonts and styles come from Google Fonts and jsDelivr. These services may see your IP address when they load. WhatsApp links open WhatsApp, which has its own privacy policy.

Your health details

Please don’t send detailed medical history through the form. Share it with the doctor in person.
